Our services are intended for patients seeking sleep-related healthcare services,
including but not limited to:
Use of our services does not create a physician-patient relationship until
an agreement for care is signed and formal services are rendered.
2. Communication Consent
By providing your contact information (including mobile number and email address),
you consent to receive communications from California Sleep, Inc., including appointment
reminders, service updates, billing information, and healthcare notices.
You may opt out of marketing or non-essential messages at any time by replying
"STOP" to any text message.
Essential communications related to your healthcare, appointment status, or
billing may still be required for your care even if you opt out of market= ing
communications.
3. Payment and Billing
Patients are responsible for providing accurate insurance and payment information.
Payment for services not covered by insurance is the responsibility of the patient.

4. Equipment Use and Responsibility
If you are issued durable medical equipment (such as CPAP machines), you are
responsible for following the usage guidelines and returning equipment if necessary
under the terms provided at the time of equipment issuance.
5. Limitation of Liability
California Sleep, Inc. makes every reasonable effort to provide accurate
services and information. However, we are not liable for delays, errors, or service
interruptions outside of our control, including technical malfunctions or third-party
service outages.
6. Privacy and Data Security
All patient information is protected according to our Privacy Policy, which can be found at www.calsleepinc.com/policy.html.
We do not sell or share your personal information for marketing purposes.
7. Changes to Terms
We reserve the right to update or modify these Terms and Conditions at any time.
Updates will be posted with a revised "Effective Date" at the top of this page.
Continued use of our services after changes are posted constitutes acceptance of those
changes.
